Japanese national fee schedule, code K082-8 robotic TKA at 42,190 points against 37,690 points for conventional K082, a premium of 4,500 points or about 45,000 yen per case at 10 yen per point. Surgical fees effective 1 June 2026. Japan is the only market in this study where the robotic increment is reimbursed, which inverts the economics: the model here is a revenue premium, not a cost offset. The facility standard sets a regulatory volume floor of 5 cases a year for new notification and 10 for renewal, so volume is a condition of payment rather than only a driver of amortisation.
